Ascenix is software for the staff who run retirement homes, assisted living, and skilled nursing. A director of care describes a job the team still does by hand, such as finding falls in progress notes and listing the follow-ups policy still requires before the morning huddle. The product reads the electronic health record nurses already use, including PointClickCare, and does not write back to the chart. From those notes it produces incident analysis, compliance checks before inspection, reimbursement for care given but never claimed, private-pay billing, and staffing, in one login. Rishi Mehta and Ayaan Esmail, the Toronto-based founders, are launching it in Y Combinator's Fall 2026 batch. They previously built Fallyx, a wearable fall detector, then moved into Scarborough Retirement Residence in 2024 and lived there for months. That home became the first customer. The months on site changed what they built. Priorities on the floor moved faster than one device, and homes were already paying separate vendors for incident reports, missed reimbursement, and scheduling. Ascenix says it is growing 15% week over week across Canada and the United States, with a team of seven. A home names the manual process it wants handled, and Ascenix assembles the workflow from blocks already tested in other facilities, fitted to that home's own policy and records. Nothing has to be installed, and nurses do not chart the same note a second time.
